What is the NCD Care Service
It allows selected diabetic and hypertensive patients from these participating facilities with a self-management tools to enter their daily measurements from the comfort and safety of their own home, office or any location to be monitored and managed remotely by their respective medical doctors in their various hospital in real time.

How do I access NCD Care?

Be recommended by Doctor
Patients must be recommended for hypertensive or diabetic patients by their treating physician from UGMC, 37 Military Hospital, or GARH

18 years+
Patients must be over 18 years old

Requires a smartphone
with Android software 7.0 and above, or iOS 12 and above

Share your name, phone number and email address
with your doctor at any of the participating facilities. Sign the consent form with your doctor at any of the participating facilities

Get notified
Patients receive an email with instructions to download the Luscii vitals app from the Play Store or App Store and log in

In-app Guide
The app will prompt patients on when to take measurements, which they can do from home and securely send to their doctor via the app

Receive timely reminders
The Luscii app will send messages to patients, such as reminders to take measurements or contact their doctor or go to the nearest hospital if needed

Continuous support
If necessary, a member from the medical team will call you directly at the comfort of your own home.
